Interview Forms
HR will not process incomplete packets.
Additionally, all recommendations must include the following:
1) Signed Committee Member Confidentiality Statements from each member.
2) Recommendation Form signed by all committee members. (Committees must be an odd number with a minimum of 3 members - all members must be a director or campus administrator.)
3) Justification/Reasons for the candidate's selection. (May be included on the Recommendation Form.)
4) Interview Summary Form. (Completed by each committee member on each interviewed applicant.)
5) Candidates responses to questions - one from each committee member for each interviewed applicant.
6) Copies of 3 reference checks - Minimum two professional with 1 of those being current or most recent supervisor (available through the Recruit/Hire System).
7) Copy of job posting;
8) Candidate interview schedule with dates and times (Be sure to indicate if a candidate cancelled or did not show. Also, indicate if any interviews were via a conference call or other means.)
Resources are available on the district's HR website under Administrator Resources. Please note we will update forms and information periodically so you should download forms directly from the website. HR hopes to eventually become paperless in this process as we begin to fully utilize the Recruit/Hire System.
