    for its Pre-K & Headstart Programs

     

    Thank you for your interest in the South San Antonio Early Childhood Program.
    Our program offers educational and family services to children who are 3 & 4 years of age.

  • The following documents will need to be uploaded during the application process. If you are not able to upload them, you will need to bring them to your scheduled appointment:

    1. Birth Certificate

    2. Valid Photo ID (parent or guardian registering child). Expired IDs will not be accepted.

    3. Immunization Record

    4.  Child's Social Security Card

    5. Proof of Medical Insurance (Medicaid, CHIP, Private Insurance, etc.)


    6. Proof of Residency (light bill, water bill, or lease)


    7. Verification of address (if proof of residency is not in the applicant's name)


    8. Disability Documentation (if applicable)


    9. Current Income (12 months of income to include: Income Tax 1040, All W2 Forms for all jobs held, last 2 consecutive check stubs, Child Support/ Financial Activity Report showing 12 months of payments, unemployment, and any other sources of income).


    10. Federal Assistance:

    • TANF Award Letter ( dated within a year of the application date)
    • Supplemental Security Income Award Letter ( dated within a year of the application date)
    • SNAP (Current Award Letter no more than a year old)

    11.  Foster and Kindship Award Letter (dated within a year of the application date)

    12. Physical Exam (most current)

    13. Dental Exam (most current)
